What are your hours of operation?
We are open from 9am to 5pm Monday through Friday. Due to surgical schedules, office hours can vary for each surgeon. Every effort is made to stay on schedule. However, emergencies do occur and we appreciate your patience and understanding in these situations.
Can I call the office at any time?
Our staff handles calls during normal office hours from 9am until 5pm, Monday through Friday. If the office is closed and you have an urgent need to reach the doctor, please notify the answering service at (847) 759-1110. Our on-call physician will return your call as soon as possible.
For all office visits, payment is required at the time of service. Co-pays and or coinsurance are also required at the time of your visit.
Additional charges may be generated for procedures and tests. We will bill your insurance carrier, however, you will be responsible for any deductible and/or co-pay amount.
If it is determined that surgery is necessary, we will coordinate payment arrangements for any procedures your insurance company determines to be your responsibility.
Payment plans can be arranged prior to surgery if necessary. We will gladly assist you with your billing questions. Please call our billing office at (847) 759-1110 between 9am and 5pm Monday through Friday.
When will my surgery be scheduled?
All surgeries are scheduled after your visit with the surgeon. Our staff will make every effort to see that your insurance company is notified and that all pre-certification requirements have been met. Please provide the most recent insurance information to ensure that accurate authorizations are obtained.
What is your cancellation policy?
If circumstances prevent you from keeping an appointment, you must call the office at least 24 hours in advance to let us know. This allows us enough time to offer your appointment to another patient. Failure to provide at least 24 hours notice counts as a missed appointment (No Show) and you will be charged a twenty five dollar ($25) fee. This will not be covered by your insurance company. If you are more than 15 minutes late and fail to notify the office in advance of your appointment time, your appointment will be cancelled (No Show) and you will need to reschedule.
If you have any questions about the "Cancellation Policy", please ask to speak with the office manager.
What is your prescription policy?
Please contact our office directly between the hours of 9am and 4:30pm, Monday through Friday, for prescription refills. Prescriptions will not be refilled after hours or on weekends. Please anticipate your needs and call early so we can assist you with your prescription needs.
Are you affiliated with specific hospitals?
Our physicians are proud to be affiliated with Advocate Lutheran General Hospital, Illinois' most nationally recognized hospital. We believe that successful treatment outcomes depend on teamwork between the surgeons, operating room personnel, excellent nursing staff and other physicians. Nurses at Lutheran General Hospital have earned Magnet Status, the highest recognition in nursing care. Advocate Lutheran General Hospital is also one of 30 hospitals in the nation to achieve exemplary outcomes for surgical patient care, including lowest post-operative complications, by the American College of Surgeon's National Surgical Quality Improvement Program (NSQIP).
Some of our outpatient procedures are also performed at Golf Surgical Center as well as Glen Endoscopy Center, depending on the preference of you and your primary care physician and the scheduling availability of the hospital.
